  • My associates and I, do accept some insurance plans.
  • One simple call to your insurance will help in getting the following questions answered for your behavioral / mental health coverage. 
  • Provide the following information for benefits & coverage verification purposes : 

 

  1. Have your insurance card with you to provide your ID number 
  2. Check In-network & Out of Network benefits with your insurance.

  4. Ask if mental health is subject to a deductible? If so how much has been met? 
  5. What is your copay per session? 
  6. Is authorization required? 
  7. Are there any limitations in the number of sessions per year?
  8. Is telehealth & virtual sessions covered by your plan?
  9. Get a reference number for your call.
  10. Email a copy of your insurance card front & back. 

Cancellation Policy 

  • We value and hold high regards in our scheduled time. In case of not being able to keep your appointment notify your clinician 24 hrs in advance. 
  • Less than 24 hours cancellation or a missed appointment is subject to the full fee of your session.
